  Consortium Agreement Request Form Use this form when applying for aid for combined enrollment at OSU-OKC & schools with whom we have consortium agreements.
  Satisfactory Academic Progress - GPA or Completion Ratio Appeal Form Use this form if you want to appeal a GPA or completion ratio decision. Please note: you cannot appeal a previously denied appeal decision.
  Satisfactory Academic Progress - Excessive Hours Appeal Form Use this form if you want to appeal a excessive hours decision. Please note: you cannot appeal a previously denied appeal decision.
  Remaining Credit Hours Extension Form This form is required to accompany the Excessive Hours Appeal form and must be completed by your Academic Advisor.
  Course Participation Form Use this form to document your attendance and/or participation in courses. You must print and complete a form for each course you are needing to document attendance on for any semester requested.
